Community Health Needs Assessments and Implementation Plans

Memorial Hermann Health System conducts a Community Health Needs Assessment for each of its facilities in accordance with the Patient Protection and Affordable Care Act to identify the community’s health needs and strengths; to prioritize health issues; to provide a foundation for the development of a community health improvement plan; and to assist community organizations in their program planning.

Most recently completed in June 2022, the analysis included a review of secondary data on social, economic and health issues in the region with qualitative information from focus groups with community residents and service providers and input from numerous community representatives.

The resulting Strategic Implementation Plans (SIPs) set hospital-specific goals, objectives and strategies that address each facility’s specific service area and populations served. CHNAs and SIPs are available for the following facilities:
